MAPP2Health Report
The MAPP2Health reports are the Community Needs Assessment for the area. The area is known as the Thomas Jefferson Health District, or Planning District 10, is composed of Albemarle County, Charlottesville City, Fluvanna County, Greene County, Louisa County, and Nelson County. The MAPP2Health core group team is composed of representatives from the Thomas Jefferson Health District, Sentara Martha Jefferson Hospital, and the University of Virginia Health System. The core group team works with organizations in each locality to get data and help with outreach. The Report is updated every four years, with supplemental data and reports in between those updates.
In the 2016 MAPP2Health Report the four major areas of need were Promoting Healthy Eating and Active Living, Addressing Mental Health and Substance Abuse, Improving Health Disparities and Access to Care, and Foster a Healthy and Connected Community.
Move2Health Equity uses these four areas as framework for our mission.
